The video explains the concept of sequential binding to help understand how psychiatric drugs work at different dosages. It uses the drug Seroquel (quetiapine) as an example. The key idea is that drugs don’t hit all receptors equally; instead, they bind to receptors sequentially based on their binding affinities. Every psychiatric prescriber should know this principle as a means to understand psych med dosing.
At low doses, Seroquel binds to receptors involved in sedation, like histamine and alpha-1 receptors. As the dose increases, it starts binding to receptors involved in mood regulation, like serotonin receptors (around 150mg). It’s not until higher doses (around 300mg) that Seroquel binds to the D2 receptor, which is responsible for its antipsychotic effects.
This sequential binding explains why Seroquel has different indications at different dosages and dosing psych meds. Low doses are sedating, moderate doses are better for mood disorders, and higher doses are needed for antipsychotic effects. The concept applies to many psychiatric drugs, where lower doses may act more like a different class of drugs until higher doses that engage the desired receptor. Understanding sequential binding provides an intuitive grasp of how dosages influence a drug’s effects.
