This is from the second Utaho no Tatari game. You can download it for free here: (play it it’s good) This event is very easy to trigger, just keep walking under each of the three torii until the fake error message pops up. You may also want to save your progress before doing this, as it’ll stay on the picture of the dead body until you close the game, loud sound and all.
