what about biologic therapy?

A 22-year-old woman presented with #bleeding per rectum, bloody diarrhea, and severe anemia. The patient gives a history of ulcerative colitis—the patient is on treatment for #ulcerative-colitis in the form of #Pentasa, #corticosteroids, and other #conservative treatments. She underwent a #colonoscopy. The scope revealed severe colitis with multiple pseudopolyps and whitish-sloughed mucosa. The extent of the inflammation is about 2/3 of the colon extending from the rectum. Multiple biopsies were taken for histopathology. The patient and her relatives ask about biological therapy; what is your opinion? How to confirm or exclude associated #GI infections? How to exclude intestinal #dysplasia? What is the #follow-up plan for this patient? Is it essential to check for #primary non-response to #TNFα antagonists?
